Your first step is to fill out our general intake form. We also have the more comprehensive intake form that you can download and bring with you on your first visit. Please fill out the form and we will get back to you as soon as possible.
For the online intake form, if the form is completed over the weekend, the intake information will be addressed with the Intake and Scheduling Coordinator on normal business days Monday thru Friday.

These will be required forms at the time of formal intake at the time of the first visit. There will be additional forms needed at the time of intake based on the assessment & services required.
